Physician Partners IPA Expands To Brevard, Features Innovative Clinical Programs

Physician Partners has been caring for Central Florida seniors in partnership with select affiliated Primary Care Physicians since 2006, and is now actively expanding into Brevard County.
Physician Partners currently serves nearly 23,000 Medicare Advantage Members through seven health plan partners across a 14-county Central Florida market
Physician Partners and its affiliated physicians are transforming the health care services platform and dramatically improving the health outcomes for Medicare Advantage Members through the development of a focused patient-centered medical home experience in the primary care clinic.
One public measure of Physician Partners’ success is the overall CMS HEDIS quality rating of 4.84 (out of 5) Stars in 2014 achieved by the more than 120 primary care affiliated physicians, giving it the highest such quality rating of any large Independent Physician Association in Florida.
