BREVARD COUNTY • ROCKLEDGE, FLORIDA – Brevard Palliative Care began accepting patients in December and has been growing steadily since to care for more and more individuals who need support in the greater Brevard County area.
Palliative care is specialized medical care that focuses on quality of life for patients and their loved ones by managing the symptoms and stressors of a serious illness.
Palliative services include pain and symptom management, advance directive planning, help with care transitions, and appropriate referral to other types of care as needed.
Brevard’s specialists collaborate with patients’ current physicians to add an extra layer of support.
Brevard’s Clinical Director, Amy Tremante, RN, BSN, CHPN, is a life-long resident of Brevard County who previously served as the Transition Specialist at Wuesthoff Hospice for nearly five years.
“I am honored and grateful to be able to offer true palliative services to our community,” said Tremante. “This program will fill a gap in the continuity of care for so many people with serious illness and chronic disease.”
Tremante is joined by Medical Director Dr. Nabil Aziz, MD, and Nurse Practitioner Amy Gill.
Brevard Palliative Care is part of LHC Group, a leading national provider of in-home healthcare. Brevard Palliative Care provides high-quality care to patients through our home health, hospice, home-and-community-based services, and facility-based care.
